Job summary

System Support is seeking an Early Intervention Service Coordinator (EISC) to act as the primary contact for families, coordinating services and supporting home visits for children with an individualized family service plan.

The EISC will collaborate with the supervisor, ensure timely assessments, and communicate across agencies while maintaining confidentiality and compliant records.

Qualifications

• Bachelor's degree required in social work, early childhood or related field.
• Criminal background check with Ohio BCI and I/Or child abuse registry required.
• Origin orientation and two years minimum of relevant experience preferred.

Responsibilities

• Collaborate with supervisor to identify learning needs and develop development plans.
• Manage a caseload of 45-60 children with an IFSP.
• Document work schedule in Outlook and track client visits.
• Achieve first visit within 14 days and complete paperwork within 7 days.
• Promote seamless care across services and maintain confidentiality.
