Job summary

Care Lync invites a dedicated Supports Coordinator to join our Early Intervention Services team in South Carolina. You will provide ongoing service coordination for children birth to age 6 and their families, ensuring access to needed services and understanding of rights, while promoting child development and family capacity.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education or Special Education, Child Development, Family Studies, or related field.
  • Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
  • CPR/First Aid certification preferred; support to obtain it upon hire.

Responsibilities

  • Serve as primary contact for families.
  • Assist families in accessing needed services.
  • Coordinate and monitor service delivery.
  • Make referrals and schedule appointments.
  • Conduct follow-up to ensure services are provided.
  • Coordinate evaluations and IFSP/FSP development.
  • Participate in multidisciplinary team meetings.
  • Help families identify goals and outcomes.
  • Maintain accurate records and documentation.
  • Ensure compliance with Medicaid and IDEA Part C.
