Job summary

Camino Real Community Services in Texas is seeking a Service Coordinator for Early Childhood Intervention (ECI) to support families and young children with developmental needs. You will coordinate services, develop IFSPs, and connect families with therapists, educators, and providers to ensure timely, holistic care.

You will document progress, maintain compliance with regulations, and advocate for families while working closely with multidisciplinary teams in a supportive community setting.

Qualifications

• Bachelor’s degree in social work, psychology, education, nursing, or related human services field.
• Experience with young children and families, preferably in early intervention or special education settings.
• Knowledge of early childhood development and developmental disabilities.
• Strong organizational and communication skills.
• Ability to manage multiple cases and coordinate services effectively.

Responsibilities

• Conduct initial and ongoing assessments to identify the needs of children and families within the ECI program.
• Develop, implement, and monitor individualized family service plans (IFSPs) in collaboration with families and multidisciplinary teams.
• Coordinate and schedule services with therapists, educators, healthcare providers, and community resources to ensure seamless delivery of care.
• Provide families with information, support, and advocacy to navigate available services and resources effectively.
• Maintain accurate and timely documentation of service coordination activities, progress notes, and compliance with state and federal regulations.
• Facilitate communication among all stakeholders to resolve issues and promote collaborative problem-solving.
• Participate in team meetings, training sessions, and professional development to stay current with best practices in early childhood intervention.
