Qualifications

Responsibilities

• Conduct initial and ongoing assessments to identify needs of children and families within the ECI program.
• Develop, implement, and monitor individualized family service plans (IFSPs) with families and teams.
• Coordinate and schedule services with therapists, educators, healthcare providers, and community resources.
• Provide families with information, support, and advocacy to navigate available services and resources.
• Maintain accurate documentation of service coordination activities and progress.
• Facilitate communication among all stakeholders to resolve issues and promote collaboration.
• Participate in team meetings and ongoing professional development.

Education

Bachelor’s degree in social work, psychology, education, nursing, or related human services field
