Job summary

Lutheran Services Florida is seeking a Family Case Manager who provides comprehensive wrap-around case management to children and families. You will establish strong relationships, ensure timely home visits, and coordinate resources to support reunification and permanency planning.

Core duties include creating case plans, documenting in FSFN within 48 hours, and collaborating with guardians, foster parents, and service providers.

Qualifications

• Minimum one year of relevant experience.
• Bachelor's degree in a Human Services field.
• Degree in Social Work preferred.
• Must be certified within one year.

Responsibilities

• Meet with children in their primary residence within 2 working days of staffing.
• Develop and monitor family case plans for reunification or permanency.
• Document activities in FSFN within 48 hours.
• Coordinate referrals and maintain community resource knowledge.
• Attend court activities and prepare required paperwork.
