Job summary

Coleman Professional Services seeks a dedicated Care Coordinator / Case Manager in Ashland, OH to support children, youth, and families with complex behavioral health and social service needs. You will help navigate systems, coordinate services, and stay connected to care, with structured supervision and coaching provided.

This is a full-time, community-based role in Akron area, offering paid training, three weeks of PTO in the first year, and wellness reimbursement.

Responsibilities

• Coordinate behavioral health and social services across providers, schools, and community systems.
• Support families with care planning, follow-up, and service connections.
• Maintain timely, accurate documentation in an electronic health record (EHR).
• Monitor safety, risk, and follow-up needs with supervisory guidance.
• Communicate regularly with families, providers, and partner agencies.
• Track tasks, deadlines, and follow-ups to ensure continuity of care.
• Participate in team meetings, training, and individual supervision.
