Integrated Services for Behavioral Health in Jackson County, Ohio, is seeking a Care Coordinator to support youth and families through OhioRISE services, coordinating care across home, school, and community settings.

The role focuses on leading teams, developing plans, and ensuring family voice and choice in services delivered, with a pay range of $20.19–$25.03 per hour based on experience and licensure.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Valid driver's license and reliable vehicle.
• Ability to travel within assigned service area.
• Commitment to HIPAA and confidentiality.

Responsibilities

• Lead, plan, and facilitate team meetings for the youth and family.
• Provide care coordination in home, school, or community settings.
• Collaborate with social service providers and families to identify needs and services.
• Maintain documentation to access and maintain OhioRISE benefits.
• Follow ISBH policies and procedures, ensuring confidentiality and professionalism.
• Travel within the service area as required.
