Job summary

Integrated Services for Behavioral Health in Jackson County, OH, is seeking a Care Coordinator to support youth and families with OhioRISE services. You will organize and lead multi-agency teams, plan care, and coordinate services in homes, schools, and communities.

The role emphasizes family voice, coordination, and outcomes, with a pay range of $20.19–$25.03 per hour based on experience and licensure. Travel within the service area is required and strong documentation is essential.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Education aligned with listed options or higher as described in the job posting.
• Valid driver’s license and reliable vehicle for in-home/community visits.
• Ability to travel within the assigned service area and meet training requirements.
• Strong verbal and written communication with solid documentation skills.
• Hi-Fidelity Wraparound knowledge is preferred (Certification provided at time of employment).
• Experience with coordinating care across family systems and community resources.

Responsibilities

• Lead and facilitate child & family team meetings for youth at home, school, and in the community.
• Provide care coordination within the family’s home, community, or preferred location.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Collaborate with families, natural supports, and service providers to meet care goals.
• Travel within the assigned service area and participate in trainings.
• Ensure compliance with HIPAA, confidentiality, and professional guidelines.
