TFI Family Services in Independence, KS is hiring a full-time Aftercare Case Manager to support children and families achieving permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll focus on risk management, provide crisis intervention, and document accurately.

You’ll meet families in person, coordinate community services, and advocate for families, with training provided.

Qualifications

• Bachelor’s degree in human services is required.
• Valid driver’s license required.
• Willingness to learn; training provided.

Responsabilités

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve concerns.
