Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager to join our team in Kansas.

You’ll support children and families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care, focusing on risk management and stability.

You’ll provide crisis intervention, coordinate community services, maintain documentation, and meet families in person to promote reunification and lasting safety.

Qualifications

• Bachelor’s degree in human services.
• Valid driver’s license.
• Willingness to learn—we’ll provide all the training you need!

Responsibilities

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve concerns.
