Job summary

TFI Family Services in Chanute, KS is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship, or aging out of care.

You will focus on risk management, provide crisis intervention, maintain documentation, coordinate services, and meet with families in person to help reunification and resolve concerns. A bachelor’s degree in human services and a valid driver’s license are required; training is provided.

Responsibilities

• Support children and families who achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve concerns.
