Job summary

TFI Family Services, Inc. in Kansas is seeking a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care.

You will focus on risk management, crisis intervention, and coordinating services with community agencies while maintaining accurate documentation and reporting. Willingness to learn and provide ongoing training are key.

Qualifications

• Bachelor’s degree in human services required.
• Valid driver’s license required.
• Willing to learn – training provided.

Responsibilities

• Support children and families who have achieved permanency through reintegration, adoption, custodianship, or aging out.
• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and coordinate with public/private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and address concerns.
