Job summary

TFI Family Services, Inc. in Kansas (Independence area) is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, or guardianship.

You will focus on risk management, crisis intervention, documentation, and coordinating services, meeting families in person to support reunification. We seek compassionate advocates who connect families with community resources, are eager to learn, and committed to strengthening

Qualifications

• Bachelor’s degree in human services required.
• Valid driver’s license required.
• Willingness to learn—we’ll provide training!

Responsibilities

• Support children and families who have achieved permanency through reintegration, adoption, or guardianship.
• Focus on risk management to help families maintain stability during the aftercare period.
• Provide crisis intervention and coordinate with public and private agencies.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families and meet with families in person to support reunification.
