Job summary

Prestera Center for Mental Health Services is seeking a Family Coordinator to support families affected by substance use. You will perform needs assessments, develop service plans, and coordinate with treatment providers to deliver integrated, family-centered interventions.

You will collaborate with the Peer Family Support Specialist to ensure a strengths-based approach, advocate across systems, and manage data for program reporting and improvement.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or a related human services field.
• Strong advocacy and service coordination skills across multiple agencies.
• Excellent interpersonal and communication abilities with culturally diverse populations.
• Experience with data management and program reporting.
• Knowledge of social determinants of health and family systems.

Responsibilities

• Conduct needs assessments for families affected by substance use.
• Develop individualized service plans with realistic goals and steps.
• Coordinate with treatment providers and community resources for integrated support.
• Advocate for families within child welfare, legal, and social service systems.
• Maintain accurate records and timely data entry for reporting.
• Collaborate with the Peer Family Support Specialist to support families.
• Provide crisis intervention and transportation assistance as needed.
