Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Mental Health Care Coordinator to assess needs, develop person-centered recovery plans, and coordinate services for children and families. You will provide skills training, crisis prevention, and documentation, while maintaining compliance with policies and laws.

The role requires collaboration with providers and strong communication.

Qualifications

• Bachelor's degree in psychology, sociology, behavioral or human services with ability to obtain QMHP-CS credential.
• Valid Texas driver's license and auto insurance.
• Certification in PMAB, First Aid, and CPR within 3 months and maintained thereafter.
• Preferred: 1 year of experience with CANS or Texas Resilience & Recovery Model.

Responsibilities

• Assess needs and collaborate to develop and execute recovery and life management plans.
• Provide skills training to improve daily living.
• Collaborate with service providers to liaise for effective care and outcomes.
• Document clearly and accurately with proper grammar.
• Maintain compliance with legal requirements and company policies.
• Meet regularly with individuals to track progress and compliance.
• Manage crisis situations as they arise and communicate effectively.
