Job summary

Mental Health Cooperative, Inc. is seeking a Care Manager I to provide comprehensive care management services for children, youth with SED and their families, and adults with SPMI.

You will develop relationships with consumers, collaborate with an interdisciplinary team, and help them achieve their goals within the community. The role requires a Bachelor’s degree in a health-related field, a Tennessee driver’s license, and a focus on coordinated, patient-centered care.

Qualifications

• Bachelor’s Degree in a health-related field required.
• Criminal justice graduates must have fifteen college hours of behavioral health coursework.
• Valid Tennessee Driver’s License and proof of insurance required.

Responsibilities

• Provide comprehensive care management services to children, youth with SED and their families, and adults with SPMI.
• Engage with consumers monthly to assess needs, develop and address care plan goals.
• Complete visits in the community (home, school, court, etc.) to evaluate needs and connect with individuals.
• Address mental health, physical health, social determinants of health, gaps in care, and ED/IP utilization.
• Provide a side-by-side approach to address barriers to engagement in treatment.
• Ensure timely documentation and meet productivity standards.
