Qualifications

• Licensed as a Physical Therapist in the state; CPR certification if required.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and communicate with physicians and families regarding progress.
• Complete timely documentation in accordance with regulations and payer requirements.
• Provide in-services and training for facility staff on rehabilitation programs.
• Assist with discharge planning and participation in quality assurance activities.
