Job summary

GRANDVIEW in the healthcare facility seeks a Staff Occupational Therapist, prn, to evaluate and treat residents, communicate with families and physicians, and maintain documentation in medical records. You will conduct in-services for staff and participate in interdisciplinary teams.

The role requires licensure as an Occupational Therapist, strong documentation practices, and the ability to supervise COTA and aides while ensuring compliance with state regulations and payer requirements.

Qualifications

• Bachelor's degree in Occupational Therapy; master's or doctorate preferred.
• Licensed to practice as an Occupational Therapist in the state.
• CPR certification, if required by licensure.
• Proficiency with Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Supervise Occupational Therapy Assistants and aides in resident care.
• Communicate progress and discharge planning with physicians, nursing staff, and families.
• Complete timely, accurate documentation per facility and payer requirements.
