Job summary

Luxor at Sayville Rehabilitation and Nursing Center seeks an experienced Physical Therapist to assess and treat patients, develop individualized rehab plans, and document progress. The PT will supervise PTAs, coordinate with care teams, and educate patients and families while maintaining HIPAA compliance.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing and records.
• Evaluations, weekly progress notes, recertification and discharge documentation.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals per patient capacity and physician direction.
• Set realistic goals, document patient status, and educate patients and families.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care and professional standards.
