Job summary

Luxor at Sayville Rehabilitation and Nursing Center is seeking an experienced Physical Therapist to assess patients and provide direct rehab services in a long-term care setting. You will establish treatment plans, set goals, document progress, and educate patients and families under physician guidance.

The role requires a PT license, experience with geriatrics, and collaboration across disciplines to deliver patient-centered care. Professionalism and confidentiality are essential.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Must have a valid professional license and registration.
• Oversee and supervise Physical Therapy Assistants.
• Geriatric/long-term care experience preferred.
• Strong documentation skills.
• Experience with evaluations, progress notes, recertification and discharge documentation.
• Collaborates with disciplines for patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctual, professional, and compliant with confidentiality.
• Familiarity with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care to evaluate medical condition and functional status.
• Develop and administer a treatment program with goals based on patient capacity and tolerance under physician direction.
• Set realistic, achievable goals, document patient condition, and educate patients and families.
• Maintain timely documentation and progress notes; manage discharge planning as needed.
• Collaborate with other disciplines to deliver optimal patient-centered care.
• Report to Rehabilitation Director and adhere to scheduling and professional standards.
