Job summary

Sayville Rehabilitation & Nursing Center seeks an experienced Physical Therapist (PT) to join Luxor at Sayville facility. You will assess referred patients, design and supervise treatment plans, and work under the direction of the physician to restore function and independence.

Responsibilities include documenting progress, evaluations, discharge planning, supervising PT assistants, and collaborating with the care team.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed and registered to practice in the state.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation writing skills.
• Ability to complete evaluations, progress notes, recertifications and discharge records.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.

Responsibilities

• Assess referred patients for rehabilitation needs and goals.
• Develop and administer treatment programs under physician direction.
• Set realistic goals and document patient condition and progress.
• Educate patients and families on therapy methods.
• Document evaluations, progress notes, recertifications, and discharges.
• Supervise and oversee Physical Therapy Assistants.
• Collaborate with all care disciplines for optimal outcomes.
• Report to rehabilitation leadership and ensure professional conduct.
