Job summary

Luxor at Sayville Rehabilitation and Nursing Center seeks an Experienced PT to assess and treat patients requiring rehabilitation services in our facility. You will create and monitor individualized therapy plans, set achievable goals, and document patient progress under physician direction.

The role emphasizes collaboration with a multidisciplinary team, supervisory responsibilities for PTAs, and adherence to HIPAA and high professional standards. Prior LTC/geriatric experience is preferred.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Current valid professional license and state registration.
• Experience supervising Physical Therapy Assistants.
• Knowledge of documentation processes including progress notes and discharge documentation.
• Familiarity with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Develop and administer individualized treatment plans with physician input.
• Set realistic goals based on capacity and tolerance.
• Document patient condition and progress; educate patients and families.
• Supervise and collaborate with Physical Therapy Assistants and other disciplines.
• Maintain confidentiality and HIPAA compliance; punctual and professional.
