Job summary

Luxor at Sayville Rehabilitation and Nursing Center in New York seeks an experienced Physical Therapist to assess and treat patients in a long-term care setting. You will develop and administer rehab plans, document progress, and educate families while coordinating with physicians and the care team.

The role requires an accredited PT program completion and a valid license, with preference for those familiar with long-term geriatric care and strong documentation skills.

Qualifications

• Graduate from an accredited Physical Therapy college or university.
• Valid professional license and registration for state of practice.
• Experience with long-term care and geriatric care preferred.
• Excellent documentation and record-keeping skills.
• Ability to coordinate with other disciplines for patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document and record patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and collaborate with the care team.
• Maintain punctuality, professionalism, and adherence to schedules.
