Job summary

Luxor at Sayville Rehabilitation and Nursing Center is seeking an experienced Physical Therapist to assess patients, develop treatment plans, and supervise PTAs in a geriatrics-focused facility. You will document patient progress, educate families, and collaborate with the care team to achieve optimal outcomes.

The ideal candidate holds an accredited PT degree, a valid state license, and has strong documentation skills. Experience in long-term care and PPS scheduling is a plus.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid state physical therapy license/registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertifications and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service and clinical excellence.
• Reliable attendance and professionalism.
• Compliance with confidentiality and HIPAA.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients and provide direct patient care.
• Develop and administer a treatment program with goals.
• Document patient condition and progress.
• Educate patients and families in therapy methods.
