Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess patients, design and supervise rehabilitation programs, document progress, and educate patients and families in therapy methods.

The role requires a PT degree, an active state license, and experience with long-term/geriatric care. Collaborative, patient-centered care and strict confidentiality are essential.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid professional license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Conduct evaluations, weekly progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Maintain punctuality and professionalism at all times.
• Perform duties as directed by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA and Residents' Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer a treatment program with goals aligned to patient capacity.
• Document patient condition and educate patients and families on therapy methods.
