Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an experienced Physical Therapist (PT) to lead assessment and treatment for rehabilitation patients at our facility. You will develop and oversee treatment plans, set achievable goals, document progress, and educate patients and families under physician direction.

The role emphasizes oversight of Physical Therapy Assistants, quality documentation, and collaboration with multidisciplinary teams to ensure exceptional patient care

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must have a valid license and registration for the state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Ability to supervise Physical Therapy Assistants.
• Strong documentation skills and record-keeping.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer individualized treatment plans with goals.
• Document patient condition and progress; educate patients and families.
• Collaborate with all disciplines to deliver patient-centered care.
• Ensure timely completion of evaluations, progress notes, and discharge documents.
