Job summary

At Peace Health Care Agency is seeking an experienced Direct Support Professional (DSP) to provide person-centered care to individuals with IDD in home and community settings.

Responsibilities include supporting ADLs, promoting independence, documenting progress, and collaborating with families and care teams. The role requires a compassionate, dependable professional able to work independently and follow individualized care plans.

Qualifications

• Previous DSP experience is required.
• Experience supporting individuals with IDD and/or Autism.
• Experience working with behavioral needs and BSPs preferred.
• Knowledge of person-centered care and individual rights.
• Strong communication and problem-solving skills.
• Compassionate, patient, dependable, and adaptable.
• Ability to assist with personal care and mobility needs.
• Ability to work independently and responsibly.

Responsibilities

• Provide direct support with ADLs, personal care, mobility, and daily routines.
• Support individuals with IDD, Autism Spectrum Disorder, and behavioral needs.
• Implement Individualized Support Plans and Behavioral Support Plans (BSPs).
• Promote independence, dignity, choice, and community involvement.
• Provide companionship and support during community activities and appointments.
• Assist with transportation as needed.
• Monitor and document progress, services, and daily activities accurately.
• Maintain a safe, respectful, and person-centered environment.
• Protect individual confidentiality and follow HIPAA and agency policies.
• Communicate effectively with individuals, families, and the care team.
