Linley Park Post Acute in Anderson, SC is seeking an experienced Unit Manager (LPN) to lead a resident care unit and ensure top‑tier outcomes. You will oversee daily operations, support nursing staff, and help maintain regulatory compliance with a focus on patient‑centered care.

You will participate in on‑call leadership rotations, collaborate with physicians and therapists, and drive quality improvement while fostering a supportive, team‑oriented culture.

Qualifikationen

• Current, unrestricted South Carolina LPN license.
• Current CPR certification.
• Supervisory or leadership experience in skilled nursing or long‑term care preferred.
• Knowledge of clinical documentation, care planning, and regulatory compliance.
• Experience with PointClickCare (PCC) preferred.

Aufgaben

• Oversee daily clinical operations of an assigned resident care unit.
• Supervise and support CNAs, Medication Aides, and nursing staff.
• Monitor resident conditions and ensure timely intervention for changes in status.
• Conduct rounds and address resident, family, and staff concerns.
• Assist with admissions, transfers, and discharges.
• Maintain accurate clinical documentation and regulatory compliance.
• Participate in care plan meetings and interdisciplinary team conferences.
• Support survey readiness and quality improvement initiatives.
• Partner with physicians, therapists, and department leaders to achieve positive resident outcomes.
• Assist with staff development, coaching, education, and performance management.
• Promote a positive, team‑oriented environment focused on resident care excellence.
