Qualifications

• Current, valid Texas LNFA license (NHA/LNFA) is required.
• 3–5 years of LNFA experience in a Skilled Nursing Facility.
• Proven ability to achieve or exceed EBITDAR budgets and financial targets.
• Demonstrated success in census development and referral relationship management.
• Strong knowledge of state and federal regulations governing SNF operations.
• Comprehensive understanding of SNF reimbursement models across multiple payors.
• Successful track record of regulatory and quality inspections.
• Exceptional leadership, communication, and customer service skills.
• Ability to work in a fast-paced environment with a strong sense of urgency.

Responsibilities

• Deliver strong EBITDAR performance through disciplined revenue management, cost control, and labor optimization.
• Drive census growth and occupancy through strategic marketing, referral development, and hospital/ACO engagement.
• Build and strengthen relationships with hospitals, physicians, case managers, and other referral sources.
• Ensure a rapid admissions process and improve conversion rates to position the facility as a preferred provider.
• Lead QAPI initiatives to improve outcomes and meet quality-based reimbursement targets.
• Oversee risk-management, survey readiness, and timely reporting of plans of correction (POCs).
• Foster a culture of safety, dignity, and service excellence for residents and families.
• Recruit, develop, and retain high-performing leadership and staff; drive employee engagement.
