The Carlyle at Stonebridge Park – Southlake, TX is seeking an Executive Director (LNFA) to lead the skilled nursing facility's operational, clinical, and financial performance. Qualifications

• Current, valid Texas Nursing Home Administrator license (LNFA) required.
• Minimum 3–5 years of experience as an LNFA in a Skilled Nursing Facility.
• Proven ability to achieve or exceed EBITDAR budgets and financial performance targets.
• Demonstrated success in census development and referral relationship management.
• Strong knowledge of state and federal regulations governing nursing home operations.
• Comprehensive understanding of SNF reimbursement models across multiple payors.
• Successful track record of regulatory and quality inspections.
• Exceptional leadership, communication, and customer service skills.
• Ability to work in a fast-paced environment with a strong sense of urgency.

Responsibilities

• Deliver strong EBITDAR performance through disciplined revenue management, cost control, and labor optimization
• Champion a service-excellence culture for residents, families, visitors, and staff
• Analyze financial reports and KPIs; identify trends and implement corrective actions
• Drive census growth and occupancy through strategic marketing, referral development, and hospital/ACO engagement
• Build and strengthen relationships with hospitals, physicians, case managers, and other referral sources
• Ensure a rapid, seamless admissions process that improves conversion rates
• Achieve positive survey outcomes through proactive oversight of survey readiness and POCs
• Ensure delivery of compassionate, resident-centered care meeting standards
• Foster safety, dignity, and service excellence for residents and families
• Proactively address resident and family concerns with timely resolution
• Build and lead a high-performing management team
• Strengthen employee engagement and reduce turnover
• Collaborate with nursing leadership to improve quality measures and financial outcomes
• Maintain proactive risk-management through systems oversight and timely reporting
• Lead QAPI initiatives to improve outcomes and reduce rehospitalizations
• Oversee discharge planning and strong care transitions
• Monitor accounts receivable aging to maintain revenue cycle health
• Represent the facility in community events to support brand and referrals
• Provide strong leadership to department managers and staff
• Recruit, develop, and retain leadership and staff
