Near North Health in Chicago is seeking an Executive Director of Population Health to lead the systems that improve the health of the population it serves. Your Role

The Executive Director of Population Health leads the systems through which Near North Health organizes and improves care across the population it serves. The role is cross-cutting by design. It spans operations, clinical programs, information systems, managed care, care coordination, and data, and it exists to connect these functions into a coordinated approach to the health of the whole panel rather than the care of one patient at a time. As a member of the executive leadership team reporting to the Chief Executive Officer, the Executive Director owns the value-based care model, the population health infrastructure, the care coordination function, and the cross-cutting programs that close care gaps at scale.

Compensation

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
