Job summary

Near North Health in Chicago is seeking a Nutritionist to join our patient-focused team. You will deliver participant-centered nutrition education, provide breastfeeding support, and coordinate referrals to WIC services to advance program goals.

Your Role

Delivers participant-centered nutrition education, breastfeeding support, and referral services to WIC participants to support program goals and objectives.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards and participate in all Patient Centered Medical Home efforts.

Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role. Candidates who meet the minimum requirements will typically start at the entry level of the range, with additional qualifications reflected in the final compensation package.
