Job summary

Evolving Solution Services is seeking a Part-Time Peer Support Specialist in Lancaster, CA, to support Transition Aged Youth (ages 16–25) with outreach, information referral, and mental health/rehabilitation services. The role includes coordinating with subcontractors to ensure client participation and successful outcomes.

Responsibilities include crisis intervention, skill training, and diligent follow-up, with data entry in EXYM and participation in outcomes planning.

Qualifications

• Bachelor's Degree preferred in social work, psychology, sociology, etc.
• Familiar with linkage services in the Antelope Valley and surrounding areas.
• Ability to work well independently and as a team member; strong interpersonal skills
• Advanced level PC skills
• Professional telephone manner
• Ability to write routine reports and manage correspondence
• Excellent prioritization and organization skills and strong interpersonal skills

Responsibilities

• Coordinate service by helping clients implement plans and link to community resources
• Collaborate with therapists and internal/external partners to ensure resources
• File client demographics into electronic profiles
• Input notes into EXYM within 24 hours
• Attend outreach activities including evenings and weekends
• Coordinate with subcontractors for timely data collection
• Attend weekly supervision sessions
• Greet clients and conduct initial intake
• Enter demographics and data in EXYM or designated system
• Maintain tidy work area for clients
• Perform other duties as assigned
