Job summary

• Bachelor's degree in a related field (preferred).
• Familiar with linkage services in the Antelope Valley and surrounding areas.
• Strong interpersonal and organizational skills.

Responsibilities

• Coordinate services by linking clients to community resources.
• Collaborate with therapists and subcontractors to ensure resources are provided.
• Document client demographics in electronic profiles promptly.
• Enter notes into Exym within 24 hours.
• Attend outreach events evenings and weekends.
• Assist with data collection and program participation monitoring.
• Participate in weekly supervision.
• Conduct initial intake and greet clients.
• Drive and handle data entry in designated systems.
• Maintain tidy work area.
• Perform other duties as assigned.
