Oak Hill is seeking a Direct Support Professional in Farmington, CT. You will support individuals in daily living activities, health and wellness, nutrition, mobility, documentation, and emergency response while working a Friday-Saturday night schedule (11:00 PM–7:00 AM) for about 16 hours weekly.

No experience? Paid training is provided. The role emphasizes compassion, adaptability, teamwork, and clear communication to promote independence and community inclusion.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to safety, health, and professional work environment.
• Commitment to continuity of care, including occasional stay beyond a scheduled shift until coverage is in place.

Responsabilités

• Assist with personal care, grooming, hygiene, and continence care.
• Monitor health information and document vital signs as required.
• Administer medication and maintain medication records per training and directives.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and provide mealtime support.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, and incidents in the EHR and comply with regulations.
• Respond to emergencies with First Aid/CPR when required.
• Communicate effectively with team, individuals served, families, and stakeholders.
