Job summary

Qualifications

• HS diploma or equivalent; excellent verbal and written communication skills and two years of driving experience required.

Responsibilities

• Provide and/or assist with personal care and emotional support to persons served; help individuals reach maximum independence.
• Support persons served in meal preparation, daily living activities, and maintain a clean, healthful environment.
• Assist with medical appointments as needed.
• Monitor and report changes in individual's condition or living situation to the Program Director and/or Case Manager.
• Maintain confidentiality of each person served information.
• Communicate with the Case Manager, Program Director and persons served regarding schedule and needs.
• Engage persons served in meaningful community/recreational activities.
• Attend trainings and mandatory meetings; complete timesheets and mileage reports on time.
• Other duties as assigned.
