Job summary

Melwood is seeking a Community Support Assistant to provide personal care and help individuals served maximize independence, Monday through Friday, 10am–4pm, 30 hours per week. You will support meal prep, daily living activities, and accompany to medical appointments, all while maintaining confidentiality and engaging in meaningful community activities.

The role is supervised by the Program Director and the Community Support Services team; a HS diploma, strong communication skills, and driving

Qualifications

HS diploma or equivalent; excellent verbal and written communication skills and two years of driving experience required.
Experience working with individuals with developmental disabilities, specifically in a community-based setting strongly desired.
May require use of personal vehicle to transport persons served; Melwood's Travel policy applies.

Responsibilities

Provide/personal care and emotional support to persons served and assist toward maximum independence.
Assist with meal preparation, daily living activities, and maintaining a clean, healthful environment.
Assist with medical appointments as needed.
Monitor and report changes in individual condition or living situation to Program Director/Case Manager promptly.
Maintain confidentiality of person-served information.
