Job summary

BHcare in Connecticut seeks a compassionate Transitional Counselor for the Young Adult Services (YAS) Residential Program to support young adults in health, recovery, and independent living. This 16-hour weekly, part-time position is scheduled Saturday and Sunday 11pm–7am and is designated as Onsite Essential Staff.

You will assist residents with daily living activities, help develop recovery plans, build therapeutic relationships, and apply Motivational Interviewing and Trauma-Informed Care.

Qualifications

• Requires associate degree or two years of college in related fields; or HS diploma with 2+ years direct care experience.
• Experience with mental health or co-occurring disorders preferred.
• Strong communication, problem-solving, organizational, and interpersonal skills; CT driver license required.

Responsibilities

• Assist residents with daily living activities such as hygiene, meals, budgeting, and community integration.
• Support residents in developing and implementing personalized recovery plans and goals.
• Build therapeutic relationships to foster independence, self-confidence, and self-advocacy.
• Use recovery-oriented approaches including Motivational Interviewing, Trauma‑Informed Care, and Person‑Centered Planning.
• Provide crisis intervention and support during behavioral health emergencies.
• Connect residents to clinical, educational, vocational, recreational, and community resources.
• Maintain documentation and electronic health records, progress notes, assessments.
• Assist with transportation, appointment coordination, and community activities.
• Monitor safety and promote a supportive, structured living environment.
