Qualifications

• Current LPN license in the state of New York.
• 0 years of experience in a long-term care setting (new graduates welcome to apply).
• Ability to work a variety of shifts, including evenings and weekends.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct patient care, including vital sign monitoring, wound care, and assist with activities of daily living (ADLs).
• Administer medications and treatments as prescribed by physicians.
• Document patient information in a timely and accurate manner.
• Collaborate with the interdisciplinary team to develop and implement individualized patient care plans.
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction.
• Perform other duties as assigned by the nursing staff.

About the Role
