We are currently seeking an honest, committed, and compassionate Licensed Practical Nurse (LPN) to join our team for Part-Time/Per Diem Evening or Night shifts.

Key Responsibilities

• Provide direct patient care and administer medications and treatments as prescribed.
• Assist with resident assessments, physical observations, and regular monitoring of vital signs.
• Collaborate closely with our Director of Nursing (DON) and interdisciplinary healthcare team members to implement resident care plans.
• Perform basic life support measures in emergency situations.
• Document resident information accurately and efficiently using electronic health record (EHR) systems.
• Provide compassionate support, education, and reassurance to residents and their families.

Qualifications

• Valid LPN nursing license in the State of New York.
• Basic Life Support (BLS) certification.
• Proficiency in electronic health record (EHR) documentation.
• Compassionate, patient-centered approach with a strong commitment to working with older adults and individuals requiring rehabilitative care.
