Job summary

Park Gardens Rehabilitation & Nursing Center is seeking a licensed practical nurse to provide compassionate geriatric care at our facility. You will support admissions, ongoing nursing tasks, and collaborate with RNs to ensure safe, effective treatments.

Responsibilities include administering meds, monitoring vital signs, documenting care, and guiding other staff while maintaining high standards of patient comfort and safety. NYS LPN licensure and BLS/CPR are required.

Qualifications

• Completion of an accredited educational program and NYS licensure as an LPN.
• Current BLS/CPR certification.
• Experience in a long-term skilled nursing facility preferred.

Responsibilities

• Assist with admission, transfer and discharge of patients.
• Participate in planning, implementing and evaluating nursing care and activities.
• Administer and record medications and treatments, including IV therapy.
• Prepare for and assist RNs and other professionals with procedures.
• Reinforce instructions to patients and families.
• Communicate vital signs and observations to physicians and supervisors.
• Record patient responses in charts and maintain a safe environment.
• Assist with unit management or take charge as assigned.
• Perform other nursing functions as delegated by Nursing Service.
• Guide technicians and aides in patient care.
• Maintain clinical competence via continuing education.
• Assist patients with activities of daily living.

Shifts Available: Fulltime/part-time/per-diem:

Park Gardens Rehabilitation & Nursing Center, hasimmediate openings for LPNs. We are looking for a talented and motivated Licensed Practical Nurses (LPN’s) to join our dynamic team. Applicants for this position, should have a passion for geriatric care and be able to work independently without direct supervision. The successful candidate should possess the desire to perform functional therapeutic activities, assist residents improve their physical and medical conditions during their stay at our facility. Responsibilities:

• Assists with the admission, transfer, and discharge of patients.
• Participates with other team members in planning, implementing, and evaluating nursing care and other patient activities, including diagnostic procedures and treatments.
• Administers and records medications and treatments as assigned, including IV therapy.
• Prepares for and assists RNs, nurses, and other healthcare professionals with diagnostic procedures and treatments.
• Reinforces, supports, and provides healthcare instructions to patients and their families.
• Communicates observations and reports patients’ vital signs to physicians, NPs, and other healthcare supervisory professionals.
• Records observations of patients’ responses to care provided in patient charts.
• Maintains environmental conditions conducive to patients’ comfort and safety.
• Assists with unit management and/or assumes charge of the unit as assigned.
• Performs other nursing functions related to patient care and nursing services, as delegated by Nursing Service Administration.
• Guides and directs other technicians and nurses’ aides in providing patient care.
• Maintains clinical competence through participation in and attendance at continuing education programs.
• Assists patients with activities of daily living.

Requirements:

• Completion of an accredited educational program and current New York State (NYS) licensure as a Licensed Practical Nurse (LPN).
• Current BLS/CPR certification.
• Prior experience in a long-term skilled nursing facility is preferred.
