Job summary

Extendicare in Houston, TX is seeking a Registered Nurse to provide direct resident care, administer medications, and document care per policies. The role reports to the DHW/DOC and includes evening hours (15:00-23:00) every other weekend over a 13-month term.

Responsibilities include care assessments, care planning, coordinating with the nursing team, supervising unregulated care providers, and ensuring regulatory compliance. Strong communication and organizational skills are essential.

Qualifications

• Current RN license in the relevant US state or province.
• Experience working with seniors and unregulated care providers.
• Education and experience in gerontology, rehabilitation, community/long term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Sound clinical knowledge and ability to delegate to unregulated care providers.
• Maintain competencies as per governing College standards.
• Skills in computer use and Microsoft Office applications.
• Current VPS within 6 months.

Responsibilities

• To complete and ensure residents’ care assessments, service plans and documentation are up to date.
• Assist with scheduling of residents’ care conferences and participate in conferences.
• Work closely with DHW/DOC to facilitate residents’ move in and transfers.
• Order and safe keep drugs and narcotics and administer in compliance with regulations and Extendicare policies.
• Facilitate and provide resident/family education.
• Supervise resident self medication assessment and audits.
• Supervise and delegate care and medication administration responsibilities to Unregulated Care Providers.
• Plan, coordinate, manage and evaluate the resident’s care plan with the team.
• Coordinate nursing team’s resident care assignments for timely delivery.
• Respond to significant changes in residents’ condition and medical emergencies.
• Act as supervisor and coordinate emergency response in absence of DHW/DOC and Executive Directors.
• Address resident/family concerns and ensure appropriate action or escalation.
• Assist in Infection Prevention and Control, Health and Safety and Quality Plans.
• Coordinate scheduling of nursing students from approved institutions.
• Uphold Extendicare values and Code of Conduct.
• Complete other tasks as assigned.
