Job summary

UnityPoint Health in Cedar Rapids is seeking an inpatient rehabilitation nurse to care for medically complex patients in our 24-bed post-acute unit. Evening shifts 3 pm to 11 pm, no weekends, with a $10,000 sign-on bonus and differential opportunities.

You will collaborate with therapists, physicians, and case managers to develop and implement patient-centered plans, promote independence, and support safe transitions home.

Qualifications

• Graduate of accredited nursing program.
• Current RN licensure in Iowa or a compact state.
• QI Credentialing certificate.
• Fluent English.

Responsibilities

• Provide comprehensive nursing care to post-acute rehab patients with complex medical conditions.
• Collaborate with therapists, physicians, case managers, social workers and the inter-disciplinary team.
• Participate in interdisciplinary care planning and communicate progress and goals.
• Educate patients and families to promote recovery and discharge planning.
• Monitor and manage patient conditions and respond to changes in status.
• Support independence and rehabilitation goals to help patients return home.
