The Christ Hospital Home Health in Cincinnati, OH is seeking a Registered Nurse (RN) to provide comprehensive skilled care in patients' homes as ordered by the physician. You will educate patients and families, ensure safety, and maintain accurate documentation to comply with Medicare and state regulations.

This full-time role requires evening visits three days per week, on-call one night per month, and participation in on-call rotations and case conferences.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Ability to stay calm, patient and accommodating.
• Detail-oriented with strong nursing judgment.

Responsabilités

• Provide skilled nursing care in patients' homes.
• Observe and monitor patient conditions.
• Perform OASIS assessments to develop patient-specific care plans.
• Administer medications as prescribed by the physician.
• Coordinate care with LPNs and aides to ensure continuity of care.
• Document all patient care and maintain compliance with agency protocols.
