Job summary

The Christ Hospital Home Health is seeking an RN On-Call to support a team of nurses and patients after hours. You will provide one-on-one patient care, chart reviews, and family education from your home when appropriate, per physician orders.

Responsibilities include OASIS-based care planning, medication administration, supervising aides, and coordinating with the interdisciplinary team to ensure high-quality outcomes for home-based patients.

Qualifications

• Minimum one year RN experience in an acute care setting.
• Home care experience preferred.
• Effective communication with patients, families, physicians and co-workers.
• Calm, patient, compassionate demeanor.
• Ability to work autonomously and manage time.
• Valid driver’s license and auto insurance in your name.
• Designated workspace at home for chart reviews.

Responsibilities

• Observe and monitor patient conditions and perform OASIS to develop an individualized care plan and adjust as needs change.
• Evaluates the treatments and medical condition of the patient according to the plan of care.
• Determines if the level of care being provided meets the patients’ needs.
• Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.
• Oversee and supervise total care of patient provided by nurse aides and LPNs.
• Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
