Job summary

Parkland Community Health Plan is seeking an experienced Registered Nurse to join the Care Coordination team as a Care Manager. The role focuses on developing individualized plans of care, ensuring quality patient care across the healthcare continuum, and communicating with patients, families, and Parkland staff.

The position requires an RN license in Texas, BLS certification, and at least two years of hospital or community-based nursing experience.

Qualifications

• Must be a graduate of an accredited school of Nursing with Texas RN license.
• Two years of hospital or community-based patient care nursing experience.
• Current BLS certification required.

Responsibilities

• Assess patients on Care Coordination team to develop a plan of care.
• Identify funding sources and potential eligibility for patients.
• Monitor progress and adjust the plan to ensure quality outcomes.
• Coordinate discharge planning and transitions with care team and family.
• Communicate plan of care to patient, family and providers; ensure documentation.
