Qualifications

• Must have RN license (Texas Board of Nursing) or Compact RN license.
• Current BLS for Healthcare Providers certification from AHA/ARC/Network.
• Graduated from an accredited school of Nursing; two years hospital or community care nursing.

Responsibilities

• Conducts assessment of patients to develop a case management plan of care with funding source determination.
• Collaborates with multidisciplinary team to implement the plan of care and monitors progress.
• Communicates plan of care to patient and family and reassesses for effectiveness.
• Utilizes care management processes to ensure quality, cost-effective care and appropriate discharge.
