Job summary

Parkland Health is seeking a Registered Nurse for Care Coordination at New Parkland Hospital. This on-site, days shift role focuses on establishing care plans, utilization management, and patient advocacy to ensure quality, cost-effective care across the continuum.

Two years hospital nursing experience and an active Texas RN license (or Compact) are required, along with current BLS from multiple sources. Works Monday-Friday, 7:30am-4pm, with PRN employment type.

Qualifications

• Graduate of an accredited school of Nursing.
• Two years hospital or community-based patient care nursing experience.
• Texas RN license (or Compact) and current BLS from AHA/Red Cross/Military Training Network.

Responsibilities

• Conducts patient assessments on Care Coordination team to develop case management plans.
• Identify funding sources and eligibility; coordinate with physicians and care team.
• Communicate plan of care to patient and family; reassess and update plans as needed.
• Collaborate with multidisciplinary team to optimize outcomes and ensure cost-effective care.
• Monitor utilization and discharge planning; resolve barriers to care and transitions.
