Job summary

Community Mental Health Affiliates, Inc. (CMHA) seeks a Part-time 3rd shift Case Manager for Respite Services in New Britain, CT. You will support clients toward independent living and coordinate care across CMHA teams.

Responsibilities include caseload management, delivering education and life-skills training, assisting with daily living tasks, and ensuring timely documentation. A Bachelor's in a mental health field is preferred; CT driver’s license and CPR/First Aid are required.

Qualifications

• Minimum of related case management experience.
• Education: Bachelor's in mental health field preferred.
• Must have valid Connecticut driver's license and reliable vehicle.
• Eligible for MANDT, First Aid, and CPR certifications.
• Experience with EMR systems (appointment scheduling, records, discharge summaries).

Responsibilities

• Maintains caseload as determined by CMHA.
• Provides direct care with education, skill building, recreation and household tasks.
• Teaches life skills for independent living, including finances and housing.
• Educates clients on emergency and community resources.
• Develops and reviews individualized treatment plans and documents progress.
• Ensures timely documentation in CMHA systems.
• Supervises self-administration of medication when applicable.
• Coordinates with treatment teams and external providers.
• Provides transportation as needed.
