Job summary

Community Mental Health Affiliates, Inc. in New Britain seeks a full-time Case Manager for the 2nd shift. You will support clients with daily living activities, coordinate transportation, and ensure timely communication across shifts to maintain continuity of care.

Requirements include a High School Diploma or GED, a valid driver's license, and willingness to obtain MANDT, First Aid, and CPR certifications. Prior case management and EMR experience are preferred.

Qualifications

• Education: High School Diploma or GED.
• Experience: 1 year in case management or related field is preferred.
• Competencies: strong interpersonal, communication, and multitasking abilities.

Responsibilities

• Provide direct assistance to clients in activities of daily living and program chores.
• Coordinate transportation and keep program staff updated on client progress.
• Supervise self-administration of medications and document shift notes.
• Maintain safety and confidentiality of client information per agency policies.
• Communicate effectively to ensure continuity of care across shifts.
